Washington Center for Aging Services (WCAS) is a government owned 259 bed long-term care nursing facility, located in North East, Washington D.C. The organization is managed under the direction of VMT Long Term Care Management Inc.
Scope of Position
Position is responsible for the entire management of the social services department in efforts to improve the quality of life for seniors through strategic partnerships with internal disciplines and external resources.
Brief Description of Duties
§Operate as a leader interpreting policies/procedures and manage social services department providing general direction and guidance to ensure optimal performance
§Coordinate activities of the social services department with other disciplines
§Successfully document and manage caseload to meet licensure/inspection standards and handle casework/resident needs by applying psychotherapeutic approaches, assisting with financial matters, administering conflict resolution and utilize community resources to enhance services for resident population.
§ Provide counseling services to family members and resident responsible parties to maximize relationship potential.
§ Coordinate and facilitate discharge planning process
§ Conduct research, prepare findings and present teachings on relevant social services topics
Core CompetenciesKnowledge of Medicaid/Medicare and all pertinent federal, local, and industry specific regulations.Computer literate Ability to ensure confidentiality of PHI and sensitive organizational dataHighly accurate, detail oriented with demonstrated organizational skillsAbility to effectively communicate utilizing diverse mediums across various levels and with assorted publics Ability to work collectively with others and ability to lead a clinical teamAbility to analyze, interpret policies/procedures and problem solve to ensure the most advantageous outcome for the facility
Minimum QualificationsMaster degree requiredLicensed Independent Clinical Social Worker or Licensed Clinical Social Worker with active District of Columbia licenseDemonstrated clinical skills and management ability; preferably in a long-term care settingValid driver’s license
