CLINICAL PHARMACIST
The Phoenix VA Health Care System is recruiting for a Clinical Pharmacist!The primary functions of the clinical pharmacist include the following mid-level provider responsibilities: patient and provider therapeutic consultations, patient medication therapy monitoring, and direct patient medication therapy and disease state management. The incumbent functions under the supervision of the Clinical Pharmacy Program Manager with oversight by the Clinical Pharmacy Specialist(s) in their assigned area. The clinical pharmacist shall routinely provide and support clinical pharmacy, pharmacoeconomic, and mid-level provider activities and is directly responsible for monitoring and managing medication therapy for assigned patients. The clinical pharmacist shall be responsible for providing medication information and drug literature analysis as requested. As a secondary function to support patient care, the clinical pharmacist will assist in the accurate and timely verification of on-line patient medication orders. The incumbent shall be responsible for participating in the educational services and activities of the pharmacy department, and the incumbent will help coordinate and participate in the conceptualization, development and implementation of all pharmacy-based medication and service-related studies and research projects as requested. Applicant(s) selected for this position will be eligible to apply for an award up to the maximum limitation under the provisions of the Education Debt Reduction Program. BASIC REQUIREMENTS: a) Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g). b) Graduate of a degree program in pharmacy from an accredited college or university. c) Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico), or the District of Columbia. d) This position requires a pre-employment physical examination. e) Proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). QUALIFICATION REQUIREMENTS: Completion of the equivalent of one year of professional pharmacy experience at the next lower grade level, GS-11, which is directly related to the position to be filled. To be creditable, the experience must have required the use of knowledge, skills, abilities and other characteristics associated with current professional pharmacy practice. Hold a BS or Pharm.D. from an accredited college or university. The experience or education must have been gained within 3 years prior to the date of appointment. The department prefers candidates with a Pharm.D degree, at least one year of residency training, and clinical experience in medical and surgical specialties.
