Position Description:This position is responsible for managing a team of licensed health care professionals that provide Care Coordination Services to the policyholders of Long Term Care Insurance carrier customers. In addition to managing a Care Coordination team the position will be expected to contribute strategically to the growth and development of Care Coordination services offered to LTC carrier customers.? Responsible for hiring, mentoring, measuring and developing the licensed health care professionals of a Care Coordination Team. This includes –? Overseeing the daily activities of the team and ensuring that customer’s Service Level Agreements are being met, standard operating procedures are being followed and caseloads are being managed with high quality at the expected volume level.? Maintaining an appropriately staffed team using capacity models and staffing plans. Managing productivity, and compensating for absences and/or reduced caseloads due to other projects.? Providing clinical leadership to team members. Providing quality control as it relates to the interpretation of clinical assessments and the development of appropriate plans of care. Ensure that customer deliverables are complete, accurate and professional.? Measuring and reporting team performance as it relates to established thresholds.
