Position Description:Care Coordination Specialist serve as initial point of contact for all long term care insurance policyholders and related care coordination calls. Callers may be individual policyholders, a family member or long term care insurance carriers seeking assistance with scheduling an in home assessment through CareScout. In addition, CCS works with contracted (registered nurses nationwide) FieldScouts® to insure visits and documentation related to benefit eligibility assessments are completed accurately and expeditiously. Reporting to the Care Coordination Team Leader, the CCS will be responsible for negotiating contracts, training and quality of the FieldScout® NetworkDuties include but are not limited to the following:· Through call center queues manage all client and FieldScout calls and forward caller to appropriate party or obtain basic information to create electronic client record for Care Support Services application/software· Support internal teams by producing and delivering client materials as requested· Verifying FieldScout Network Provider information and update database as appropriate· Complete daily tasks to meet internal and external service level agreementsRequired attributes of a Care Coordination Specialist include:· Prior experience with proven commitment to customer service · College graduate (BA or BS) or 2 years customer service experience· High integrity and an eagerness to learn· Professional, upbeat, friendly, goal oriented with a strong ability to work efficiently in a fast-paced environment· Excellent communication skills, including the ability to respond and resolve customer concerns quickly. Knowledge of long-term care a plus· Reliable with solid work ethic and ability to work independently and within team · Proficient with basic computer applications such as MS Office, email, desktop faxing and other office procedures · Interested in personal and professional development in the long term care industry We seek the brightest and best candidates – this is an excellent growth opportunity.
