ASF was founded in 1985 and is the largest provider of support services to people living with HIV/AIDS in Orange County. Our mission is to prevent the spread of HIV and improve the lives of men, women and children affected by HIV/AIDS. ASF is searching for a Nurse Case Manager to assess the needs and coordinate the care of people living with HIV/AIDS.
Nurse Case Manager
ACCOUNTABILITY AND FUNCTION
Reports directly to the Supervising Nurse Case Manager. The NCM works within the interdisciplinary model to provide nursing case management to a primary case load of clients and acts as a secondary case manager to those clients being case managed by social services staff. Direct contact with clients and client systems may occur in the client’s home, ASF offices, hospitals and community agencies.
EXAMPLES OF SPECIFIC RESPONSIBILITIES Assess and evaluate prospective clients for the various programs available to the client. Develop, implement, monitor, and modify a service care plan through an interdisciplinary team process. Maintain effective communication with the client’s support system which includes, but is not limited to, attending physician, family members, other service agencies. Make appropriate referrals to community resources. Monitor all services provided to the client and assure that all available resources are being utilized. Maintain current documentation of work and provide timely submission of statistics and reports. Participate in all case management, nursing and staff meeting.
REQUIREMENTS
2-Year Degree Required, 4-Year Degree Preferred. Must be licensed as a Registered Nurse by the State of California and have either two years experience in community nursing or one year experience in community nursing and one year of clinical nursing experience. Thorough knowledge and understanding of HIV/AIDS. Skills in verbal and written communication and the ability to plan and organize multiple work tasks are essential. Must possess good interpersonal communication skills. Computer experience is a plus but is not essential. Bilingual (Spanish/English) ability is a plus.
